Scholarship Recipient AgreementIn consideration of the scholarship provided by Tip Top Tux, Al’s Formal Wear and Dream Dress Express the undersigned Recipient hereby acknowledges and agrees to the following conditions: The scholarship offered may not be exchanged for cash or its equivalent. The scholarship offered is not transferable to another person. The scholarship will be paid directly, via check, to the accredited college or university to which the recipient has been accepted and been admitted. Verification of acceptance and admittance is required prior to the scholarship payment being applied. In the event the Recipient does not attend an accredited college or university within 12 months of being awarded this scholarship, for any reason whatsoever, the awarded scholarship shall lapse and no scholarship will be awarded. The Recipient is responsible for the lawful reporting and payment of any applicable taxes, whether local, state or federal, that may be applicable to the award of the scholarship. The Recipient agrees to attend a ceremony hosted by Al’s Formal Wear, Tip Top Tux and/or Dream Dress Express to recognize all Recipients and take photographs. The Recipient agrees to the use of his/her name and or likeness in any advertising or promotional materials by Al’s Formal Wear, Tip Top Tux and/or Dream Dress Express without additional approval or additional compensation.
